Using the resources listed above to write a clear problem statement for your logic model. Your problem  statement should briefly explain what needs to change: why is there is a need for an intervention? Your problem  statement answers the question, “What problem are we working to solve?” Please include: who is your target  population, what is the problem, impact of problem, and cause of the problem. Do not tell me anything about how  your organization is going to solve this problem. This is a descriptive paragraph about your problem based on facts  and stats in the CHIP/CHA reports.  Keep in mind, this statement should take into consideration your region’s priority area, the specific population you  are attempting to help, the ONE goal within your region that your organization has chosen to focus on, your  evidence-based source, and your organization’s mission statement. Here is an example (do not copy) of a  descriptive problem statement:      Answer: Based on the Community Health Assessment (CHA) report for the East Region, vulnerable families in  the East region don’t have access to healthier foods that are essential for good nutrition. Make sure key terms (as shown in topics below) are bolded throughout your essays. Please see rubric for grading criteria. Chapter 5: Making Good Management Decisions: WORD COUNT = 594 DESCRIBE two programmed decisions and two non-programmed decisions you are likely to make in your unique program. DESCRIBE two problem-solving decisions and two opportunistic decisions that you are likely to encounter in your unique program. CHOOSE one type of participant approach (autocratic, consultative, group) to decision- making that you are likely to use in your program and EXPLAIN why you would choose that approach for your program participants over the other approaches. One programmed decision our program would likely make is the amount of money a new employee would receive. This would be programmed by the policies that dictate pay ranges. These decisions would be made in our program because we would be needing part-time and full-time employees to help run our program. Another programmed decision would be deciding what days nutrition and physical classes would occur. This would be programmed because our previously made decision was to have 3 nutrition and physical classes held at a center a week. One nonprogrammed decision we would make would be choosing the right food donor that would provide us with the most sufficient amount of food that is needed to feed the people in our region. Since we want to have 3 donation partnerships, choosing the right 3 is a nonprogrammed decision. One more nonprogrammed decision we would possibly make is if one of our food warehouses were to malfunction and our food supply getting spoiled leading to a shortage of food, we would have to decide how to obtain more food and what to tell the people trying to pick up food. One problem-solving decision we would most likely encounter is deciding who would be a backup driver/transporter if the first transporter were to unexpectedly not be able to drive. Another problem-solving decision we would possibly encounter is deciding to have a backup food supply if we were to have a shortage of food at one of our food donation centers. One opportunistic decision we would possibly encounter is having the opportunity to have the necessary equipment and/or supplies at a better, more favorable price. ForYou do not need to do all 4. You can pick a maximum of 2 from this list. Conducting the process evaluation first of all is done in order to determine whether a program activity has been implemented as planned and if there are any outputs. The steps to conducting process evaluation begin with reviewing of the activities and the outputs of the programs logic model. The process is then followed by using the logic model to identify the relationship between the elements used for intervention, then evaluating the resources and processes required for implementation, followed by developing questions concerning the process evaluation to make sure every step was followed and nothing is missing, then now choosing an evaluation method, followed by selecting the programs resources to answer the questions about the evaluation process then finalizing the whole process of evaluation. (You defined the process, but you need to put examples of how we would use process evaluation in our specific program. Provide specific examples in our program.) An outcome evaluation process involves measuring the effect of the program in the targeted situation by looking into the progress of the outcomes that the program is required to address.  In order to start on the process of outcome evaluation, we will review the outcome components of our programs logic model. Doing the outcome evaluation process begins with assessing the outcomes that have been achieved, then use the outcome logic model to determine the outcome roadmap, followed by being objective in order evaluate our outcome findings then finally calculating the cost-effectiveness as part of the outcome evaluation. (you have defined the process really well but it needs to be applied to what our program has done in terms of outcomes that we evaluated. Provide specific examples.) The stakeholders at this point are the people who are invested in the program we are taking. These stakeholders include the management, the staff handling the program, the programs partners, the agencies funding the program and the coalition members. The stakeholders that can be involved in the process evaluation include the management, the programs staff and the agencies funding the program. We involve the management because they need to know the program activities that have been implemented and the outputs that are likely to be received. The purpose of involving the programs stuff in process evaluation is because they are the core directors of the programs and in order to get correct output the staff needs to understand how to implement the programs activities. The agencies that are funding the program need to know because the process of evaluation requires financial support and they need to get informed about anything concerning the program in order to finance it. (you should also include the families we are helping as they are the most important stakeholders, and include how we are helping them.  also include more real examples of what w
