Health promotion diagnosis (also known as wellness diagnosis) is a clinical judgment about motivation and desire to increase well-being. Obesity is defined as abnormal or excessive accumulation of fat which might present a risk to one’s health. According to the Center of Disease Control and Prevention, obesity is a serious chronic disease, and the prevalence of obesity continues to increase in the United States. Obesity is common, serious, and costly. This epidemic is putting many American families in a difficulty position, affecting overall health, health care costs, and productivity. The houstonstateofhealth.com in 2019 Sunnyside had a percentage of 46.6\% of adults who were overweight and obese.  Overweight and obesity among adults in Sunnyside is higher than the city of Houston by 30.3\% and 28\% respectively and among African American than Hispanics and Whites.  People choose what to eat based on a variety of considerations—health goals, tastes developed in childhood or later in life, food prices, income available to spend on food, and the need for convenience.  During this powerpoint we will discuss the community assessment, aggregate, health diagnoses, plan, interventions, and finally the evaluation for the priority diagnosis. Community Assessment Geographic Area and Target Population There are approximately 17,751 residents Median age 32 years old 47.41\%: Males 52.59\%: Females Obese persons in this area are our target demographic. Vivian- There are 17,751 residents in Sunnyside, with a median age of 32. Of this, 47.41\% are males and 52.59\% are females. US-born citizens make up 87.07\% of the resident pool in Sunnyside, while non-US-born citizens account for 2.21\%. Additionally, 10.71\% of the population is represented by non-citizens. A total of 13,685 people in Sunnyside currently live in the same house as they did last year. Community Resources Sunnyside Multi-Service Center Community Garden Teaches aggregates about vegetables grown in the garden. Aggregates  plant, water and harvest from their community garden. Builds community sustainability and food independence. Expands knowledge of food, culture and ways to prevent food waste. Independence creates a desire to harvest and prepare fresh fruits and vegetables. LaQuita S- Growing your own vegetables provides a more diverse and healthy diet. The community that gardens together will have food forever.  The knowledge  Aggregates that live within the community that have limited access to  Fresh fruits and vegetables at a reasonable price are allowed to break the mold and expand by growing food that continue to replenish itself at a cheaper price than buying the produce . In Houston, a “super neighborhood” is a geo- graphically defined area where residents, civic organizations, institutions and businesses work together to identify, plan, and set priorities to address the needs and concerns of their com- munity. The boundaries of each super neighbor- hood rely on major physical features such as bayous or freeways to group together contigu- ous communities that share common physical characteristics, identity or infrastructure. Sunny- side Super Neighborhood will hereinafter be re- ferred to as “Sunnyside”. It is the intention of the Houston Department of Health and Human Services (HDHHS), in devel- oping health profiles such as this, to promote a better understanding by local residents, commu- nity-based organizations, community leaders, medical providers, and the public health commu- nity of the unique character and circumstances of our various communities, and to draw atten- tion to those matters that contribute to the great- est of health disparities among the citizens of our growing, culturally and ethnically diverse city. This profile also represents an effort on the part of HDHHS to provide a “baseline” of indicators of health in our communities, against which fu- ture trends in conditions can be measured and monitored, and appropriate public health ac- tions, taken. However, the research and views expressed in this paper are those of the individual researcher(s) and do not necessarily represent the views of the Baker Institute. “Sunnyside and South Park Comprehensive Needs Assessment Data Report” Sunnyside and South Park Comprehensive Needs Assessment Data Report 2 Background In a unique collaboration between Rice University’s Baker Institute for Public Policy, the Houston Area Urban League, the Robert Wood Johnson Foundation’s Interdisciplinary Research Leaders program, and Sunnyside and South Park leaders, this “Sunnyside Strong” survey was conducted from October 2018 to February 2019 in the Sunnyside and South Park neighborhoods of Houston, Texas. Because we examined Sunnyside and South Park jointly, hereafter we refer to both areas as “the neighborhood” or “Sunnyside.” Sunnyside, established in 1912, is the oldest historically African American community in South Houston, and has a unique “rurban” style, with a mix of rural and urban structural characteristics. Small, tidy row homes are interspersed with large, empty lots where horses sometimes graze. Gentrification has just begun in Sunnyside, which has a desirable, close- in location, and current residents are keen to revitalize the neighborhood but also keep its heritage intact. To aid their efforts, this survey was conducted to provide local leaders and policymakers with data about neighborhood concerns, well-being, and strengths. Specifically, this survey included a random sample of 417 households within eight randomly selected Census block clusters in the neighborhood. The survey was conducted by community members who were trained in survey administration and research ethics. The community researchers were paid a living wage and were an integral part of the research team. We used a community-based participatory research approach, which was essential to ensuring the research process was equitable, and included the voices of community members throughout. We also established a community advisory board (CAB), which held researchers accountable to the community; the CAB informed survey design, survey administration, and the dissemination of research findings. Ultimately, the team reached 417 heads of household, representing 1,312 total residents. In all, 24\% of sampled residents agreed to participate and completed the 40-minute long survey. Because of their resilient and introspective spirit, Sunnyside community leaders have declared a call to action: create a safe neighborhood for Sunnyside’s children to learn in quality schools and improve safety, housing, jobs and infrastructure for ALL residents. Like other predominantly low- income African-American and Hispanic neighborhoods in Houston, Sunnyside faces a number of challenges as a result of decades of injustice and political neglect.
