IHP 430 Final Project Guidelines and Rubric
Overview
As a student of healthcare quality management, it is vital that you are able to identify problems that arise in healthcare organizations and propose strategies for
their improvement. A critical part of this process requires you to be familiar with quality and accreditation standards and navigate the communication channels
of the organization.
For your summative assignment, you will identify a departmental problem within a healthcare organization and develop a collaborative performance
improvement initiative to address it. Ideally, the proposed evidence-based solution will serve to improve the departmental problem, thus contributing to the
overall success of the healthcare organization. The project is divided into three milestones, which will be submitted at various points throughout the course to
scaffold learning and ensure quality final submissions. These milestones will be submitted in Modules Two, Four, and Seven. The final product will be submitted
in Module Eight.
In this assignment, you will demonstrate your mastery of the following course outcomes:
• Evaluate appropriate methods of healthcare data collection and interpretation for informing organizational decision making
• Assess healthcare performance improvement initiatives for addressing gaps in organizational performance
• Evaluate requirements of current quality and safety initiatives for how they promote the culture of safety in healthcare orga nizations
• Formulate communication and teamwork strategies in quality management that engage diverse stakeholders within healthcare orga nizations
• Evaluate information management systems and patient care technologies that promote healthcare quality

The organizational problem is medical errors. Medical errors are associated with diagnostic procedures, equipment use, and medication errors. Medical errors fail to meet the requirements of quality and other requirements of regulation because they can lead to incorrect or incomplete patient care (Tawfik et al., 2018, p. 1573). It can also cause inter-departmental conflicts, communication failures, and budgeting issues. Medical errors can result in poor patient care quality and increased mortalities.
Inter-departmental conflicts can arise when there is a discrepancy in how different departments handle medical errors. For example, if one department is consistently making errors while another department has no errors, this could lead to conflict between the two departments. In addition, communication failures can occur when there is a lack of communication between different departments or members of the same department (Ahmed et al., 2019, p. 7). This can lead to employees confusion and frustration and may lead to errors. Finally, budgeting issues can arise when the cost of addressing medal errors is higher than the budgeted amount. This can lead to difficult decisions about allocating resources and ultimately impact the quality of care.
Medical errors can be challenging to address in a healthcare organization. Medical errors are among the causes of mortalities in the United States of America. The consistent causes of medical errors can be challenging to determine. Nevertheless, these errors are associated with serious problems in public health. The major error types include omission and commission errors (Rodziewicz et al., 2021, p. 28). Data from the organization’s internal systems may show that there have been a high number of medical errors in the past.
Medical errors have been addressed in the past in various ways, such as through the education of healthcare workers. The education of the healthcare workers increases awareness and promotes their practice in administering healthcare services to patients (Ahmed et al., 2019, p. 11). Close supervision of the healthcare workers and increasing training while recruiting new staff also reduce medical errors. Offering training programs helps the newly recruited staff fit into the healthcare organization system and reduces their chances of making errors in the organization. Medical errors have also been addressed by improving communication between the medical staff and the healthcare departments (Gautham, 2020, p. 62). This reduces gaps and ensures effective communication among the department and the healthcare workers.
Several relevant quality initiatives, standards for accreditation, safety, and compliance, and promote safety culture within the organization. For instance, the Joint Commissions National Patient Safety Goals includes a goal to minimize the medication errors risk. In addition, the Inst3

One of the most common problems in health care is Medication error; it is any adverse incident that can cause inappropriate medication use or patient suffering, either such error comes from the health care professional, patient, or the manufacturer. Untoward medical errors and underreported medication errors result in notable morbidity and death. Medication errors are the usual causes of death related to health-related events. These errors can occur during prescribing, preparing, dispensing, administration and monitoring. Factors associated with medication errors involve medications with indistinguishable names, drugs that are not usually prescribed, drugs to which patient may be allergic, medications that need testing to ensure proper therapeutic levels are maintained, illegible handwriting, and confusion over similar packaging among products or errors involving administration (Aronson, 2009)
To reduce medication errors and improve the health outcomes of clinical care. An improvement plan must be established through quality improvement strategies like preventing use of look-alike and sound-like drugs, enhancing medication management, and reducing the kinds and amount of medications stored outside the pharmacy. Other quality improvement strategies include establishing an electronic medication tracing process, a tool that processes management in handling physicians orders, developing an online query system of appearance that helps nurses check medications that were ordered but temporarily stopped by a doctor, and enhancing finances in information technology equipment for nursing care, for instance, personal digital assistants and mobile nursing carts and use barcodes, scanning barcodes play an important role in checking right drug and dosage form(George et al .,2019)
In addition, label the drugs right, keep medications in their original designated containers, keep the information sheet that come along with the medications, and ensure the five rights of drugs administration to make sure correct drugs are prescribed to the right patient in the right passage via the right route and right time and have second medical practitioner read it back to ensure accuracy (George et al., 2019)
The data determinants of success include reduced medication administration errors, reduced medication-related adverse events, improved departmental care coordination, and improved electronic medical records documentation. Most interdepartmental communications take place over the phone, but memorandums are used when information must be communicated in writing; the interpretation and judgment of results are typically presented in the reports discussion section. If the plan were successful, the organization would monitor its financial implications by having essential financial statements, namely profit or loss statements and the balance sheet, showing how the organization is performing financially. The organization would also monitor its financial implications by developing inventory records, which would give them an idea of equipment and other healthcare consumables bought and how they were used to improve healthcare quality (Gartengerg, Prat, & Serafeim, 2019). The inventory also shows the number of wasted materials or if any of the equipment was broken or lost at any point. An inventory record would enable the organization to decide how much to buy during the next batch and avoid wasting money on over-buying consumables.

The current information management system would contribute to the plans success by ensuring seamless communication between healthcare professionals and reducing medical errors. The current information management system would also ensure better record keeping since medical records are the only evidence and way of communication to ensure treatment is performed correctly (Fadhil, Mahmood, & Ahmed, 2021). Finally, the current information management system would ensure more efficient operation management through collecting and accessing healthcare information.
The current organizational processes that will help the plan to be successful
The first current organizational process that will aid the plan to be successful is putting the healthcare team first by ensuring everyone understands, believe in, and participate in the change. The second organizational process to help the plant succeed is using a change management model, which will help the healthcare leaders connect the change strategy to action, increasing the possibility of success (Jabri, 2017). The final process is making the change process exciting and compelling by ensuring healthcare professionals better understand the rationale behind the change when the organization prioritizes resolute, clear, and consistent communication.
How the plan will be communicated among departments
The plan will be communicated among different departments through email or the organizations website. Every department will receive a memo informing them of the organizations plan and set a date to meet each department to tell them the plan.
