Your idea/project coming forth from the Gemba walk is ‘effectively’ communicated to management via the SBAR tool. The idea/project contributes to continuous Improvement (Kaizen) of the selected process. That idea/project contributes to continuous Improvement (Kaizen) of the selected room cleaning process. 😊 Short-term Executable Project Supported by the Gemba and Data Collection Environmental cleaning is critical in preventing healthcare-associated infections (HAIs) and, in turn, save saves costs for hospitals. The Newborn/Infant Intensive Care Unit (N/IICU) environmental hygiene in specifics to room cleaning entails appropriate cleaning and sanitization of surfaces and equipment within the unit. Equipment and surfaces remain classified as critical, semi-critical, and non-critical. Critical equipment touches the vascular system, and semi-critical equipment is equipment that affects non-intact skin and mucous tissues (Allen, Hall, Halton, & Graves 2018). Non-critical touches the skin, and they include environment surfaces such as floors, furniture, walls, bedpans, intravenous poles, blood pressure cuffs, and stethoscopes. This My short-term idea/project for supporting Kaizen efforts of the room cleaning process involves implementing a multi-modal bundle strategy in the N/IICU at Children’s SBAR PROPOSAL 3 Hospital of Philadelphia to improve the environmental hygiene of room cleaning as a measure to prevent cross-transmission risk and ecological contamination. During the Gemba walk, a takeaway uncovered revealed that the cleaning process within the unit could improve to achieve Kaizen in efficiency. What about Kaizen in effectiveness? Reflect on that knowledge of what effectiveness entails; what efficiency entails. According to Allen, Hall, Halton, and Graves (2018), “the multi-modal bundle strategy considers the five key variables needed to improve cleaning practices: the product used, the procedure, equipment use, type of surface, and the contamination level of the room or unit.” In addition to the five variables, effective communication with the personnel qualified for cleaning is also a fundamental process requirement when implementing Kaizen. The bundle approach entails specific components, including establishing policies and procedures for the operation of room/unit cleaning, opting for proper cleaning, decontamination products, and educating staff with SBAR while also executing no-touch unit disinfection (Lee, Dong, Lim, Poh, & Lim, 2016). The effectiveness of the strategy Kaizen efforts will be evaluated in termswith measurements of changes in the rates of HAIs, cleaning performances based on UV lighting emitting devices, staff awareness, and costs (Staines et al., 2018). The project will consist of a 30-days pre-invention phase and a three-month intervention phase. You measure the difference between original outcomes of the process against Kaizen efforts of the process where there are OUTCOMES PLUS S Situation A cleaning bundle has been designed and implemented in the N/IICU at Children’s Hospital of Philadelphia to reduce the risk of transmission of HAIs and environmental contamination. Situation - Provides a concise statement of the opportunity/opportunities for KAIZEN of the process selected. SBAR PROPOSAL B Background 4 👍 During the Gemba walk within the unit, observed takeaways reflected ways to improve the cleaning process, and promote Kaizen. In effect, a multi-modal bundle strategy will get implemented as a short-term project to involve the units staff communication and improve the cleaning process. The approach remains organized based on a scientific structure to influence the shift from evidence into practice. Background – Includes pertinent and brief information related to the situation. 1) Name of the process 2) Steps within the process 3) Outcomes of the process A Assessment The situation assessesMy assessment is that the N/IICU cleaning process does not match the best available evidence concerning healthcare facility hygiene practices. Other findings??????? Assessment - analysis and considerations of options — what your preliminary analysis uncovered. Clearly identify the above as opportunities for effectiveness; if you have identified outcomes of the existing process, you then can project outcomes Plus. For efficiency, if you have identified steps/actions of the existing process, you then can project revised steps/actions. This is value-added information for management. From your analysis of the process, you generate ideas for possible short- and long-term projects that support the Kaizen effort. R Recommendation The multi-modal bundle strategy recommends that the units cleaning SBAR PROPOSAL 5 programs should be planned and expedited at the hospital level, including cleaning schedules that specify methods, frequency, and staff responsibility. The strategy also recommends that high-touch surfaces should be disinfected at least twice per day and low-touch surfaces should be clean daily, after the discharge of patients. What is the assertive recommendation here that management needs to understand and hopefully buy into? Is there multi-modal training to be provided to staff? Are there costs of obtaining this multi-module strategy? Recommendation/s - action requested — ideas turned into a short-term project. **** This is the most important part of the SBAR communication**** What effects do the short-term ideas/projects have on resources of the organization? Be sure to associate with any of your recommendations, a subsequent pull (strain) on resources of the organization. These will draw keen attention from management who you are communicating your ideas to. Take the lead, clearly identify to management the specifics that pull (strain) on resources! Brandy, Conducting a Kaizen effort on a selected process brings forth outcomes that result in improvements (addressing effectiveness). Conducting a Kaizen effort on a selected process brings forth steps/actions within the process that can be improved (addressing efficiency). The short-term ideas/projects support the Kaizen efforts of the process selected for the Gemba Walk. SBAR PROPOSAL 6 Grasping the understanding of SBAR knowledge positions you to ‘apply’ it to real situations which is where the value emanates. Incorporating my feedback (assignments, classroom) and using critical thinking skills, nurtures that application. You need to provide concreteness and objectivity to this SBAR communication tool in order for it to become an effective communication tool in communicating short-term ideas/projects that will support Kaizen effort of the room cleaning process. Reflecting on my constructive feedback (assignments, classroom messages), your Gemba walk being conducted at the operational/tactical level will bring forth Kaizen opportunity, and set the stage for a Capstone project bringing forth Kaizen opportunity at the strategic level. Possible topics to consider: intrapreneurship, innovation, effective communication, customer service, transformation, and carrying out the purpose/mission, core values of the organization. You have the opportunity to engage in the actual real experience of conducting a Gemba walk on a ‘specific’ process that supports a continuous improvement (Kaizen) effort. Been there, done that! You have the opportunity to engage in applying an effective communication tool (SBAR) by communicating ideas/projects to management that support continuous improvement (Kaizen) effort of the process selected.
