Discussion: Case Study: Building the
Counseling Relationship
This week, you will interact with three case studies. The case studies include working within
a mental health counseling office with an individual client, a private practice office with a
couple or family, and in a school counseling setting. (Please note: You will complete each
case study but only focus on one case study for the Assignment.) As you meet your client,
consider your own identity and how your identity may impact the counseling relationship.
Finally, consider the issue(s) the client(s) brings to counseling and explore multicultural
counseling skills to work ethically and effectively with your client(s).
Directions for Discussion:
The following interactive case study will assist you with using some of the knowledge and
awareness you have gained over the first 7 weeks of the course. Interact with the case studies
and analyze your responses, while considering how your cultural identity might impact the
counseling.
The interactive case studies provide you with options of how to interact and engage with the
client. You are encouraged to choose both options to understand why one option is more
culturally sensitive than the other option.
Throughout the case study, you will be asked a few reflective questions. You will not be
assessed on your responses to the questions but, instead, are required to take time to
thoughtfully answer each question. You are able to revisit the case study as often as you like.
Following the case study, you will engage in the Discussion Board.

Hunter Crane
Hunter is a freshman in high-school, and is attending therapy for his mother. The last time
Hunter attended therapy was seven years ago. Hunters behavior has changed drastically, as his
mom says that his grades are suffering tremendously as well as his attendance in school. When
asked about any type of social concept, Hunter replies that people suck. His mom has drug
tested him, yet he has passed. He has stated that he feels like he raised both himself and his
sister, as his father was completely absent and mother worked two jobs. Hunter was also abused
by a trusted family member, physically and possibly sexually.
Developmental Crisis
The developmental crisis that Hunter may be facing is dealing with depression as he is going
through a large identity development and puberty. Hunter has experienced a hard life, and is in a
phase where he may be experiencing various emotional and physical changes. Hunters mom has
mentioned that he may be experiencing depression, as his behavior has changed
drastically. Hunter is also skipping school, having his grades drop, and isolating himself from
his family and peers. This depression may have stemmed form Hunter going through
puberty. Hunter has expressed little interest in social concepts, and mentioned that people
suck. This may be because Hunter feels different. Being different is a social liability and can
lead to peer rejection. Early maturing boys show less competence than their on time peers.
(Broderick and Blewitt, 351).
Risk Taking Behavior
The risk taking behavior would be Hunter not caring about the grades he makes and his
excessive absence in school. Hunters grades will affect his future, both the educational and
career path that he may choose for himself. If Hunter continues to skip school, he will also be in
trouble with the law- as well as his mother. His mother will be held accountable for Hunters
actions, because he is an adolescent. This behavior can also stem into other behaviors, either
criminal or personal. Adolescent depression is a serious and common disorder. Though
adolescent males are less likely to report depression than females, they have serious risks
associated with the disorder- like suicide, substance abuse and illegal activity. (2017).
Protective Factor
One protective factor that may help Hunter would be his therapy. By attending any type of
mental health service, a client is demonstrating a protective factor. The therapist mentions that
Hunter smiles when art is brought up. By knowing this, the therapist knows an interest of
Hunters. The therapist can use this interest to stem a connection to Hunter, demonstrating
another protective factor. If Hunter continues to open up to the therapist, a very close bond
occurs and Hunter begins a healthy trusting bond with the facilitator.
Impact of Risk-Taking Behaviors
1. Hunters attitude may influence his relationship with his mother, which may cause tension
between them. Trust can also be broken, causing the two to continue to have disagreements and
issues.
2. Because of Hunter missing school, criminal charges can occur. Hunters mother would be
held solely responsible, though Hunter face various other consequences.

specifically on the complex stage of adolescence, and the role of systems influences. The

Steven Jeong
Steven is a 17-year-old male of Korean Descent who resides with his Mother and
two siblings: a sister (12) and a brother (10). His brother has autism. Steven’s parents divorced
about six years ago and he describes his relationship with his mother as good when she isn’t
nagging him and not very good with his father. Steven had a standard visitation schedule with
his father but seems to see him less of recent. Steven just went through a breakup with his
girlfriend who he states cheated on him with his best friend. As a result, he quit the baseball team
as his best friend played on the team with him.
Steven’s mother presented him for counseling based on the fact that she found
two empty bottles of vodka in his closet and a bottle of prescription medicine she could not
identify. She is worried that he may have a substance abuse problem. Steven admits to some
alcohol use, but not that it is a problem.
The Developmental crisis that Steven is facing may be substance abuse and possible
depression. Steven is only seventeen and has been in a somewhat serious relationship. Early
relationships in teens are linked to depression according to Broderick & Blewitt (2020). Steven
stated that he and his girlfriend broke up due to her cheating on his with his best friend. He then
quit the baseball team to avoid contact with his best friend and stating he had lost interest in
baseball. The loss of his friend and best friend coupled with his quitting baseball and substance
abuse are definite triggers for depression. According to Kassis, Artz & White (2017), “adolescent
depression is commonly associated with psychosocial impairment in multiple domains, including
family, school and interpersonal relationships” (p. 722). Friendships are one of the most
important risk factors in adolescents and are more important than teen-parent relationships in
personality development and risk-taking behavior (Broderick & Blewitt, 2020).
As Steven’s counselor I would also assess his depression and suicidal thoughts to ensure
we are not missing anything. He is a teen boy of Korean descent which increases his risk of
suicide as does the break-up and fight with his best friend and the lack of harmony in his familial
relationships. Suicide is the second leading cause of death at his age according to Kassis, Artz, &
White (2017). While Steven did not make any comments about suicide, one can assess this
quickly and easily and it is good practice.
Steven’s mother found two empty bottles of vodka in his closet along with pills which
Steven admitted in counseling he used. His risk-taking behavior is alcohol and drug use. Risk
taking behaviors such as these are normal in the teen years. The teen brain according to
Broderick & Blewitt (2020) is open to new experiences and learning as well as being more
vulnerable to adversity (p. 345). Further, Broderick & Blewitt (2020) state that the majority of
the evidence about teen brain development shows that the subcortical areas of the brain including
the amygdala and hippocampus mature faster than the prefrontal cortex which explains their
sensation seeking behavior. According to Broderick & Blewitt (2020), “risk taking is a
developmentally appropriate propensity as it serves autonomy related goals” (p. 408).
A few protective factors for Steven would be his mother’s involvement and his
relationship with her as well as his admittance to counseling. Parental involvement for
Steven may be both a protective factor and risk factor, but more information is needed.
According to Broderick & Blewitt (2020), a lack of parental involvement has been linked to
risky behavior in adolescents and those who perceive their mothers as warm were less likely to
lie to them and thus the parent was able to provide a more supportive role. Steven has a troubled
relationship with his father which would be a risk factor but seems to have a positive relationship
with his mother which is a protective factor. There is some evidence according to Broderick &
Blewitt (2020), that teens engaging in pseudo adult behaviors such as drinking and drug us is a
means of coping with anxiety and depression. They also may be seeking to fit into a peer group
or be recognized as an adult. This is often brought on by a lack of connectedness with adults or
parental presence in the home (Broderick & Blewitt, 2020). Steven’s relationship with his father
may play a role in this behavior.
Another aspect of personality development that should be addressed in counseling with
Steven is his Korean heritage. This was not brought up, but the Counselor should check with
Steven to ensure that he is comfortable in his own skin and not facing any issues in regard to his
racial and ethnical identity formation. We learned in Multiculturalism that often we, as
counselors, have a hard time noticing or put aside another’s racial or ethical dilemmas due to a
lack of understanding or knowledge and that we should address our shortcomings with the client
to clear the air and make sure we are not missing anything or showing any microaggressions or
ignorance.
The impact of Steven’s behaviors on his family and himself are an increased risk of
substance abuse problems and depression. Steven’s drug use is exasperating for his
mother. According to Broderick & Blewitt (2020) , forty (40) percent of teen’s parents feel
helpless, rejected and regretful of decisions made.
