There are various problems that affects us in the community where we live. Some of these
problems are health related and they can be prevented through offering some teachings concerning
the issues. Primary prevention or the health promotion is therefore a concept that is carried out in
the community to help in helping to effectively control the problems that are affecting us in the
community. In the community where I come from is susceptible to diseases such as breathing
problem and lung diseases. Generally, the community is susceptible to respiratory diseases and
some diseases such as cholera and typhoid owing to the nature of the environment. The program
will thus, be vital since it will help in enveloping some of the ways through which these infections
can be reduced or completely eradicated.
Epidemiological Rationale for Topic (Statistics Related to Topic):
Cholera and Typhoid are the Gastrointestinal diseases are regarded as some of the diseases that
can easily result in death in the community. As a matter of fact, over the past months, there have
been a lot of cases of people reporting to the hospital having the signs of the mentioned diseases.
Out of the people coming to the hospital with the signs sixty percent of the people are diagnosed
with the mentioned conditions. The trend in which people contact the disease is worrying and
there is a need to look into the matter and the first step of looking into this matter is by coming
up with a community prevention program or the health promotion program
Teaching Plan Criteria
Your teaching plan will be graded based on its effectiveness and relevance to the population
selected. This assignment uses a rubric. Please review the rubric prior to beginning the
assignment to become familiar with the expectations for successful completion.
Nursing Diagnosis:
Clinical diagnosis of the situation that the community is in showed that there are increased chances
of people contacting the disease. The water that is used is dirty and is not coming from a wellguarded source. There are a lot of dirty dust that that are finding their way into the water which
may increase the rate of infection. Moreover, there are also likelihood of the dirty contaminated
water mixing with the water that is regularly used by the people thus, increasing the chances of
contaminations. Thus, the prevention education will elaborate on what is needed to be done by the
people around the community to ensure that they do not fall victims of the impending dangers.
Readiness for Learning: Identify the factors that would indicate the readiness to learn for the
target aggregate. Include emotional and experiential readiness to learn.
Several factors can be used to determine whether the community involved is ready to learn and
receive the preventive techniques or not. In order to determine this, a monkey survey was
conducted that would give overview information concerning the readiness of the community to
learn. For this case, the community showed great interest of the plan that was to be undertaken
and many people if not everyone, supported the assertion. For instance, many accepted that the
program is a good one and will help them to conquer the diseases. The community were ready to
mobilize one another to the learning to ensure that each and every one get to understand the
concept. Another way to show that the community were ready to learn was the inquisitive nature
of the people in the community, this was a show that there is something they were missing that
the program would help in impacting on them
Learning Theory to Be Utilized: Explain how the theory will be applied.
The learning theory that will be utilized in this case to ensure that people in the community change
their way of living in order to meet the health requirement is that transformational learning theory.
According to this theory the learning process is aimed at ensuring that the community is
transformed to a healthy community. Through the preventive teachings on the measures that the
community should adopt and the ones that they need to drop, the community will be transformed
to healthy community.
Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include
the appropriate objective number and rationale for using the selected HP2020 objective (use at
least one objective from one of the 24 focus areas). If an HP2020 objective does not support your
teaching, explain how your teaching applies to one of the two overarching HP2020 goals.
One of the goals of the Healthy People 2020 is that there is a need to eradicate all the illness
factors that are likely to affect people and to help people in staying healthy. Through prevention
practices, the community will be able to achieve this goal. Another goal is to create a social and
physical environment that promotes the general health of the people. Through creating awareness
by utilizing this program, the society will be thinking on ways of improving the health of the
people.

The Alma Ata’s Health for All focused on the primary health care as a way of meeting the health
care for all. In so doing, they focus on the community participation and handle the community
needs and priorities. HP2020 objectives is also to ensure the creation of a healthy community
