Community Health Nursing
Purpose:
The purpose of this assignment is to provide an opportunity for students to work collaboratively while applying community health concepts and the nursing process to the care of a population.
Course outcomes:
This assignment enables the student to meet the following course outcomes:
1. Provide comprehensive care with increasing autonomy to individuals, families, aggregates, and communities in a variety of health care settings based on theories and principles of nursing and related disciplines. (PO 1)
2. Integrate clinical judgment in professional decision making and implement the nursing process in the community health setting. (PO 4)
4. Communicate effectively with client populations and with other healthcare providers in managing the healthcare of individuals, families, aggregates, and communities. (PO 3)
5. Practice in established professional roles to provide cost‐effective, quality healthcare to consumers in structured and unstructured settings. (PO 7)
6. Demonstrate leadership skills and collaborate with consumers and other health care providers in direct care or delegation of responsibilities within all levels of healthcare. (PO 2)
7. Accept accountability for personal and professional development as part of the life‐long learning process. (PO 5)
8. Incorporate evidence‐based practice in the provision of professional nursing care to individuals, families, aggregates, and communities. (PO 8)

Preparing the assignment
Follow these guidelines when completing this assignment. Speak with your faculty member if you have questions.
1) Student teams of three to four persons will form either by faculty assignment or self‐selection.
2) The team will conduct a community assessment that includes a windshield survey.
3) The presentation will be no longer than 15 minutes in length, with an additional 5 minutes for answering questions from the audience.

5) Ideas for obtaining additional demographic data include but are not limited to the following:

b. Aggregate (Target) Population ‐ 10 points/10\%

• Includes a thorough description of the aggregate population.
• Aggregate population is based on three or more elements or risks that impose a negative impact on the health of the community, identified in the community assessment.
• Identifies gatekeepers or key informants who will assist the community health nurse in gaining access to the population of interest.
c. Community Health Diagnoses ‐ 10 points/10\%
• Includes two community health diagnoses using the data from the community assessment.
• Includes one wellness diagnosis.
• Diagnoses are listed in the order of priority justified by the data findings and analysis.
• The diagnoses consist of four components: the identification of the health problem or risk, the affected aggregate, the etiological statement, and the support for the diagnosis (Nies, 2019, p. 102).

So--the Golden Corridor. I grew up in Chicago and have lived here for about two decades now. This was the first time that I’ve heard of the Golden Corridor. The Golden Corridor is the area where I-90 goes through. It’s Jane Addams Memorial Highway now---used to be known as the NW Tollway. The area in red is called the “gold” mine of economic profit for the communities in the area (Thayer, 2017). It has the different Fortune 500 companies as mentioned in this slide. We all know that Schaumburg is the home of many business such as the Motorola Headquarters.

As far as school is concerned, Schaumburg is in School District 54--known for the largest school district in Illinois. School District 54 connects more than 15,500 students with 2,000 staff members with a commitment to ensuring the academic success and social-emotional well-being of each child. The district includes 28 schools – the Early Learning Center, 21 elementary schools, 5 junior high schools and one kindergarten-eighth grade community school – which serve families from the communities of Schaumburg, Hoffman Estates, Elk Grove Village, Hanover Park, Roselle, Streamwood and Rolling Meadows (School District 54, 2020). District 54 has received 9 Blue Ribbon Awards from the U.S. Department of Education (School District 54, 2020).

As far as transportation is concerned, they have the Woodfield Trolley. It goes around the major landmarks of the city such as Woodfield Mall, IKEA, Streets of Woodfield, Roosevelt University, and the major hotels such as the Hyatt and the Renaissance Hotel (Village of Schaumburg, 2020). DART is the Dial-A-Ride Transportation. You can call them about 90 minutes before and you can get picked up and dropped off wherever you’d like in the Schaumburg area. It is $2. The only caveat is there is no fixed route, so you might have to wait for about an hour to get picked up.

The Schaumburg Police Department serves and protects the community. On the Village of Schaumburg website, you can file a crash report online or contest a ticket. You can text 9-1-1 if necessary. Photos or videos cannot be sent at this time. The city of Schaumburg has five fire stations. They respond to more than 9100 calls annually (Village of Schaumburg, 2020). The Village of Schaumburg charges for ambulances if needed. For a Basic Life Support, they will charge $381 for residents, $657 for nonresidents. In addition, they will have to pay $7.46/mile (Village of Schaumburg, 2020).

The aggregate population we decided to focus on is the elderly. The elderly includes those aged 65 years or older. In Schaumburg, there are 10,951 seniors (World Population Review, 2020). Out of the 10,951 seniors, 32\% of them are living alone, which can create some problems for them and the city of Schaumburg. In general, the elderly persons are in the Erikson stage of integrity vs. despair. This stage is when they look back on their lives and decide if they feel accomplished or not. Many elderly persons view their lives as successful if they had a family, job, and are happy. If the elderly person does not feel as though their life was successful, this would lead to despair.
There are many risks for the elderly population. Many times, the elderly start to slow down and cannot do as much as they were once able to. Many elderlies are retired, so they do not have to go to a job every day like they once did. With the elderly slowing down and not working, it can lead to risks such as obesity, poor living situations, and low income.
To determine the best data about this population and to come up with solutions to the risks, the gatekeepers include the elderly themselves, the Schaumburg, Illinois Senior Guide, and assisted living facilities in Schaumburg. These are the gatekeepers because we are able to get the most information out of them about the aggregate population, the risks they face, and how they can be dealt with.

COMMUNITY HEALTH DIAGNOSES
Diagnosis 1:
Risk for overweight/obesity related to average daily physical activity is less than recommended for gender and age, sedentary behavior occurring for >2 hr/day, high frequency of restaurant or fried food, and portion sizes larger than recommended.
Wellness Diagnosis 2:
Readiness for enhanced knowledge related to influenza vaccine.
We determined two priority risks for the elderly population in Schaumburg. The first risk for the elderly is overweight/obesity. The elderly persons typically are not able to move as much as they once did. Many elderly persons believe it is not safe for them to exercise, so they live a much more sedentary lifestyle than before. This can lead to many problems, which is why we thought it should be one of the priority diagnoses.
The second priority diagnosis we chose to work on was the influenza vaccine. The elderly can benefit from receiving the flu vaccine because many times they have comorbidities and are immunocompromised. Sometimes the elderlies are also not informed why it is important for them to receive the vaccine or that they even should receive the vaccine. It is important for the elderly to receive the flu vaccine and get the education on it in order to decrease their chance of getting sick.
PLAN FOR PRIORITY DIAGNOSIS #1
Long Term Goal:
Senior citizens will be encouraged to participate in the monthly exercise activities hosted by the Senior Center of Schaumburg two times a week for at least 30 minutes.
Short Term Goals:
Senior citizens will eat a well balanced meal at home instead of going out to eat one night a week.
Senior citizens can achieve community involvement and socialization by attending the monthly exercise activity hosted by the Senior Center as well as by exercising with peers will help senior citizens achieve their goals.
The Older Adult Community Market provides fresh fruit, vegetables and non-perishable groceries to help senior citizens achieve their goal of eating a well balanced meal.

Long Term Goal:
Senior citizens will be encouraged to attend medical seminars hosted by the Senior Center for education on influenza vaccine and tips on how to prevent getting the flu annually before flu season. Senior citizens will also be encouraged to visit the clinic to receive influenza vaccine annually.
Short Term Goals:
Senior citizens will receive influenza vaccine before the start of flu season.
The plan allows citizens to attend a medical seminar to ask questions about the vaccine as well as receive tips on how to prevent getting the flu as well as tips on how to treat symptoms.
Going to a clinic allows citizens to receive flu shot without appointment or hassle.

Educate individuals, families, and schools in Schaumburg about healthy food choices and factors affecting long term weight management through social media, distributing flyers and pamphlets at schools, mall and the library.

Host health fairs at Schaumburg library every Saturday to provide free BMI and weight checks, modification of social and economic policies in an attempt to reduce the populations exposure to the environmental causes of obesity (e.g., mandated changes in food composition, regulation of food advertising, and food labeling)

Primary:
Immunization for the influenza virus to all residents of Schaumburg
Educate residents about the importance of hand washing and covering coughs and sneezes.
Secondary:
Provide early treatment to those residents who show signs and symptoms of influenza such as fever, cough, runny nose, sore throat, or fatigue.
Tertiary:
Recommend and provide supportive measures including antipyretics, antivirals and adequate hydration.

EVALUATION FOR PRIORITY DIAGNOSIS
Exercise and balanced diet can reduce weight, which will reduce the risk of the client suffering from diseases such as CAD, hypertension, and myocardial infarction.
If older adults can implement these habits, obesity will decrease in this group in a few years.
Flu vaccine can protect older adults from the virus and early treatment can prevent developing any complication such as pneumonia.
Increased socialization with people in same age group will help decrease risk of depression, isolation, and help in dealing with loss of loved ones.
These interventions will help in reaching goals discussed earlier in the PowerPoint. If clients adhere to these intervention, the difference can be noticed in two years.

EVALUATION FOR PRIORITY DIAGNOSIS
Some of the measures used to evaluate meeting these goals are decrease incidence of obesity, decrease eating out in restaurants, increase participation in events hosted in Senior Center, increase in the number of older adults getting flu shots each year and attending medical seminars.
The community nurse together with volunteers will organize events and keep track of participation. They will also work with local government and healthcare facilities to track how many people have received flu shot and incidence of obesity in the community.
