WORKSHEET 10.1A: Summary Questionnaire Use the filled-out Worksheet 10.1B in the book as an example to follow as you complete this questionnaire. 1. What is the identity of your organization, and what is its mission? My organization is known as Georgia Community Foundation. We purpose to focus on youths and families specifically on increasing parental involvement in child care and development, preventing substance abuse, as well as reducing the number of school dropouts. As an organization, we are aware of parental involvement for children to have a sense of identity since infancy and enables parents to instill values in their children. Additionally, it improves student behavior in the classroom which highly reduces the rate of dropping out. Provision of care from parents also reduces dependence on drugs as a means to fight stress, depression, and anxiety. Our mission is to provide high-quality counseling services to our clients, unique grant and scholarship programs for our youth, and bold initiatives to handle our current and future community needs. 2. What is the proposed project (title, purpose, target population)? Project title: Youth Alliance against Substance Abuse Target Population: The Youth in Georgia. Purpose: This project purposes to train and educate the youth about the adverse effects of drug and substance abuse. It also aims at raising funds for the youth to invest in different projects that they consider significant in addressing their community needs. This will serve to turn the youth from drugs and focus their efforts on bettering themselves and the community at large. The awareness created will also help the youth know more about what they are risking in terms of health and safety due to the abuse of drugs. On the same note, youth councils will be elected to oversee the use and management of the donated funds. This will include the money ventured in the training programs as well as the ventured projects. 3. Why is the proposed project important? Substance abuse is linked to many acts of delinquency and crime in society. Most of our youth indulge in drugs because they have no alternative and this project is one of the best strategies to divert their minds into something positive and useful. With proper training, our youth will have an opportunity to think bigger about what they can do and be by molding their psychological and emotional well-being. They will also get role models to guide them through the withdrawal and recovery process. Additionally, the youth empowerment projects will equip the youth with important knowledge and skills that they can utilize all their life. Hopefully, the projects will render some of the youth self-employment or more suitable for employment by other businesses and organizations. This will eventually lead to the overall improvement in security and safety for the entire community since the youth can rely on themselves to put food on their table. 4. What will be accomplished by this program or project during the time period of the grant? By the end of the first year, we plan to have reached out to most of the youth particularly the ones from minority groups, and encouraged them to join our program. This will include getting them into a shelter where they can be fed and be able to attend our programs with ease. Also, since we already have some volunteers, counselors, and mentors who are willing to positively impact our youth, the training, and education programs will commence soon after. By the end of the grant period, we will have completed the training program and funded some of the projects. The funds will also be used in apprenticeship and other programs to ensure that our youth can effectively deliver on the different projects. The money obtained from the projects will also be used to fund more projects in the future. We are positive that these projects will get most of our youth off the streets and see them transform into successful people. 5. Why should your organization do the project (credibility statement)? Georgia Community Foundation has undertaken similar programs and projects in the past with incredible success as we managed to get some of our youth into formal employment programs. We also cut drug abuse by 80\% among the youth who joined our programs and even obtained some awards as proof of our hard work and commitment. We also have highly qualified staff members as well as volunteers and counselors who have previously worked with young adults who are drug addicts. I am also very passionate about transforming the lives of our young adults because I was once a beneficiary of such a program. Additionally, our organization has a great track record with our former grantors and we guarantee that your experience with us will be the same. We believe in ourselves and we ask you to believe in us as a great non-profit organization that aims at changing the lives of our youths for the better. 6. How much will the project cost during the grant time period? How much is being requested from this funder? The training and education program is expected to cost approximately $150 per individual where the majority will go into housing and food. This will cost about $75000 for 500 youths who are expected to successfully go through our program in Georgia. This is only a small fraction as compared to the amount spent by the government on drug control. Moreover, more funds will be a requirement for the initiation of community-based programs. The amount will vary depending on the project. However, the total amount required for the new projects will be about $1 million. While factoring in any extra requirement, this organization will require $1.2 from our funder for the successful completion of the project. Winning Grants Step by Step, Fourth Edition. Lyles Mark Knapp HUM5060 07/07/2021 Youth & Families Services: School Drop-Out Prevention; Increasing Parental Involvement; Violence Prevention; Substance Abuse Prevention In the first week, Georgia Community Foundation Inc. was selected as the focus organization for the proposal. The organization’s information can be accessed through the company website, http://www.georgiacf.org or the URL, https://www.guidestar.org/profile/58-1960821. Georgia is one of the states affected by high school dropout cases, violence, and drug and substance abuse. As a result, the choice of the Youth & Families Services: School Drop-Out Prevention; Increasing Parental Involvement; Violence Prevention; Substance Abuse Prevention option was based on the needs of the community. Georgia Community Foundation Inc. was chosen due to its influence on the states different communities this far. Georgia Community Foundation Inc. works to promote Georgia education and facilitate other charitable activities in the State (Candie, 2021). As a result, Georgia Community Foundation Inc. was seen as a good choice for the project. Since a large concentration of the proposal is on improving the peoples livelihood, such an organization like Georgia Community Foundation Inc. was the best fit to work on the project and implement it to success. To come up with this proposal idea and HSO, a search process was conducted. The first activity was to identify the challenges which the people of Georgia face. To understand these challenges, I used different researches on community activities in Georgia and the social interaction of the people of Georgia. Additionally, as a resident of Georgia, I understood some of the challenges that the people were facing, which needed urgent interventions. The challenges of the people thus resulted into the choice of the option of proposal idea to pitch. The second step was to identify the different organizations that work in Georgia and their activities. This was to identify some of the organizations which could fit the proposal idea to solve the real challenges of the people of Georgia. The organization search was conducted on GuideStar. Three organizations operating in Georgia were identified. The mission and activities of the organizations were checked, and Georgia Community Foundation Inc. was the best match. Some challenges were faced during the selection process. One of the challenges was identifying the exact activities the different organizations had undertaken and proved their effectiveness. Getting to know the effectiveness of a project may require one to have background information from where the project was undertaken and how it is influencing the people. Also, it was hard to predict whether the organizations would positively react to the proposal if handed over to them for implementation. Reference Candie. (2021). Georgia Community Foundation, Inc. The organization can be found through its website, http://www.georgiacf.org or the URL, https://www.guidestar.org/profile/58-1960821. The companys mission is to promote and support different activities in Georgia, including education and charitable activities, to improve the lives of the people of Georgia (Candie, 2021). The organization offers philanthropic advice to donors and the people who are targeted by charitable activities. Georgia Community Foundation Inc. ensures that charitable activities in Georgia are well-coordinated, and by linking with the donors and the recipients of charity, the coordination is well assured. Positive impacts are felt on the lives of the people. The operation of this organization is thus interesting and directly impacts the lives of the people, especially those in the low social class. Proposal Georgia Community Foundation Inc. can improve its services to the Georgian community by focusing more on the youths and their families. The rate of school dropout is still high in Georgia among the youths. The school dropout in Georgian is still above 15\%, which is a high number and needs some intervention (Governor’s Office, 2019). Georgia Community Foundation Inc. can give some concentration to tvghe young generation to ensure a 100\% transition in education in Georgia and that the graduation rate is at 100\%. To achieve this, Georgia Community Foundation Inc. needs to involve all stakeholders concerned with education matters in Georgia. One of the most important stakeholders to involve are the parents. The parents will need to be trained on how they can handle the education challenges of their children and advise them on the best way to get the best education. Parents have also to be educated on how to invest in the education of their children. Georgia Community Foundation Inc. should also come up with a program of helping families that cannot cater to the education of their children fully in Georgia and offer them partial scholarships or full scholarships and sponsorships to partake their education to graduation level. This will reduce dropout rates among those in the low economic class. The other challenge which needs to address to reduce the rate of school dropout is violence. Family violence reduces the chances of graduation among children. Families who are involved in constant violence cases have little concentration on the education of their children. Georgia Community Foundation Inc. can come up with a program to educate families with constant violence on how to handle differences and live in harmony considering their childrens lives. Georgia Community Foundation Inc. also needs to address the issue of substance and drug abuse. This is one of the main causes of school dropout in Georgia. Young people need to be educated on how they can refrain from drugs and concentrate on their education. All this will be achieved by Georgia Community Foundation Inc., giving more concentration to the youths and families and investing more in ensuring that the number of school dropouts is decreased overall in Georgia by increasing the graduation rate. Its components include inputs, actions, outputs, results, and impacts. Fort Bend Womens Center is using this logic model from its program to assist survivors of sexual assault, including domestic abuse, to achieve safety and autonomy. Inputs usually include the resources its organization needs to influence or fulfill its purpose. For FBWC, program participants, money, facilities, program partners, and personnel are included in the input. Activities include the inputs or resources of the organization as well as the program. FBWC activities include education, domestic violence, sexual as well as assault programs. FBWC events. The outputs also include the measuring indications of the progress of the program. The results tend to indicate that the inputs are used and that the actions are effectively carried out. The output of Fort Bend Womens Center includes participation, educational programs given, training programs supplied, community-based services offered, assisted sexual assault as well as domestic violence survivors, as well as the number of stakeholders. In addition, the results include the results of the program activities. FBWCs results include improved self-confidence among survivors of sexual abuse and domestic violence, decreased homelessness as well as reduced sexual violence. In conclusion, the effects include the projected change resulting from the actions of the program after seven to 10 years.
