The current state of the health care system in the city
While appreciating the massive improvements that the city has been making in its healthcare system, it is notable that there are emerging health issues within the city. One of the most pressing emerging health issues that have changed the health situation in the city is the rise in the number of mental health cases. In the recent past, the city has witnessed an increase in the cases of drug abuse. This development has led to the development of other health complications in the city, such as addiction. Many young people in the city of Joshua Tree are living with addiction, and this has had adverse social, economic, and political impacts on the city (Happen et al., 2012). While substance abuse and addiction are major mental health challenges to many cities in the United States, it is necessary to note that the city has limited mental health facilities and professionals. Notably, mental health is a serious health challenge that requires the establishment of highly trained mental health care providers, including nurses and other healthcare professionals. Additionally, mental health facilities are also unique in many ways. The absence of these healthcare professionals and facilities to handle mental health cases is a massive threat to the city's healthcare system.
A Proposed improved health care service
The proposed healthcare improvement that the city of Joshua Tree needs to make is to introduce a sub-department in the department of health that is dedicated to mental health challenges in the city. The primary reason for having this department created at the local authorities is that the problem exists at the local level, and it is only the local authorities that are based placed to address the challenges. The main role of this sub-department is to have strategies in place to address substance abuse and associated mental health challenges. The treatment of mental health challenges is an incredibly complex process that requires a lot of variables to be established. The creation of this sub-department will not only look at the treatment of mental health complications in the city of Joshua Tree but also consider the most suitable mechanisms to address combat substance abuse rather than focusing only on treatments (Smith et al., 2014). The development of strategies to combat substance abuse is likely to be much more beneficial than the utilization of treatment approaches only.
How would this improve the community?
The introduction of this department is likely to have a significant impact on the health status of the city of Joshua Tree. Like any other society, an increase in mental health cases is likely to have adverse effects on the social and even economic aspects of the city. Mental health illnesses destroy families, especially by occasioning divorce. Many individuals whose partners have mental illnesses are likely to seek a divorce since they may not bear with the situation. Besides, it is notable that the treatment of mental health challenges is costly, and it may drain the little resources that families may have. These resources could have been used to pursue other beneficial economic ventures (Gauffin et al., 2013). Thus, the creation of this sub-department is likely to have very important roles to play in the improvement of the health of the community while at the same time promoting the social and economic aspects of society.
Another potential benefit of this improvement is the enhancement of the fight against substance abuse in the city. It is important to realize that substance abuse is one of the major causes of mental health problems in the city. The creation of this sub-department is likely to lead to the enhancement of the fight against substance abuse in the city. The city is facing significant mental health issues as a result of a tremendous increase in substance abuse. Thus, one of the most important courses of action is to address substance abuse challenges in the city.
Finally, the creation of this development is aimed at facilitating the training and recruitment of sufficient healthcare professionals to handle the increase in the number of people living with mental health challenges. While appreciating the fact that there are some mental health professionals in the City of Joshua Tree, the number is not sufficient. Besides, the sub-department will oversee the establishment of sufficient mental health facilities such as hospitals and other necessary equipment. As indicated earlier, there is a difference between mental health hospitals and other hospitals. It is beneficial to make sure that there are healthcare organizations that are created solely for mental health patients.

Introduction
The current healthcare systems are faced with several problems, many of which have been tackled but continue to mutate into other forms. Technology has been the backbone of the majority of solutions to healthcare problems. Some of these solutions have been used to enhance service delivery in terms of information management, while others have been used to improve treatment options available like disease diagnosis. The state of healthcare in Joshua Tree City, as earlier reported, is stable and only needs a few improvements of here and there. Given that the suggested improvement involves a comprehensive roll-out of mental health to address the increasing cases of mental illness in the city. Given the sophisticated nature of the project -which involves massive grassroots implementation spearheaded by local government public health departments, there are three identified processes that will be implemented alongside to enable efficiency in service delivery as well as enhancing treatment options. These three processes include Electronic Medical Record/ Electronic Health Record (EMR/HER), e-Prescribing, and Referral generation and management.
Electronic Medical Record/ Electronic Health Record
In past years, medical care has gone through monumental transformations, given the complexity characterizing the problems in healthcare. Several technologies have been used to address these problems. One of the most popular technologies in healthcare has been the Electronic Medical Record and Electrical Health Record. These two technologies have a big role to play in the role out of mental health, Joshua Tree City.
Electronic Medical Record
An electronic medical record can be described as a digitized version of the traditional manual filing system of medical records. EMR serves purposes such as a systemic collection of patient health records using digital technologies to store them for easy retrieval. The following are some of the benefits that the health system in the city will benefit from after the implementation of the EMR. Enhancement of the overall quality of care within the practice offers a given time track on data, consumes less space as well as administrative resources, and easy identification of due patients for preventive screenings and checkups (Henry et al., 2016). Others include the provision of support for an array of discrete care activities, including laboratory ordering, electronic subscribing, monitoring, amongst others. An EMR also will issue accurate, up to date, rapidly retrievable information sources regardless of the place and time as long as the correct procedures of doing so are adhered to. One major challenge with the EMR is that information does not move easily from where they are stored to where they might be needed for use. The handlers must print them for sharing, thus reducing the technology to more or less the same as the traditional file system of keeping records (Shickel et al., 2017). For this reason, a more advanced version of EMR came into play; that is Electronic Health Record (EHR).
Electronic Health Record
The EHR purposefully came to do what the EMR could not do; that is to collect patient information and securely store them or share them across various healthcare organizations in line with standards and procedures put in place. This will particularly enable the city’s mental healthcare program to receive information related to cases locally present and find better ways of handling them. At the end of the day, the city’s mental healthcare will mark significant improvements across all local governments. Some of the common patient information occasionally captured and shared through the EHR include but not limited to progress notes, vital signs, medications, diagnoses, imaging reports, past medical history, allergies, and lab data.
Some of the benefits attributed to this technology include the sharing of multimedia content such as medical imaging results to various destinations using the correct procedures of doing so. It also gives the patient a role to play in the management of their own medication as they can be logging into the record interface and interpret their lab results while also comparing them with previous ones over the last one year or so and do it anytime and from anywhere. The EHR also offers more efficient storage and retrieval of information far much better than what EMR has been offering (Henry et al., 2016). The most important advantage that this technology offers healthcare is the ability to link records to sources of current clinical protocols and related research. On the side of healthcare professionals, the EHR offers the decision support systems which help them with diagnosis and prescription decisions. Finally, EHR brings together different healthcare organizations and help them to automatically share thousands of information and also help them update information while also standardizing off patient care and services.
The Relevance of EHR/EMR in the City’s Mental Healthcare Project
In summary, both the EMR and EHR will play a central role as far as the city’s mental healthcare program is concerned. With the increase of drug abuse problems in the city, cases of mental illnesses are equally expected to go up. Also, the cases are not concentrated in one particular locality but are spread across the jurisdictions of all local authorities in the city. The high numbers of the cases to be dealt with plus problems wider geographical coverage even makes the use of these two technologies more profound. The EMR will serve to collect all relevant patient information and keep them safe for use within each facility while the EHR will integrate all the EMRs in the city, thereby enhancing the management of referrals as well as improved care delivery.

Referral Generation and Management
Another important automated process in that the city’s planned mental healthcare provision will be the Referral Generation and Management system. At the grassroots will be care facilities which will be responsible for the treatment of first occurring cases as well as suspected cases. These facilities will form the first tier of the city’s mental care structure (Naslund et al., 2016). Right on top of them will another tier where one facility will combine four local authority jurisdictions. Finally, there will be two main referral facilities located in Joshua Tree City. These two facilities will be the final referral facilities in the city and will be responsible for referred cases emanating from the second tier facilities. e-Prescribing
e-Prescribing is the first growing technology in the healthcare industry because of the advantages it comes, such as the framework that allows physicians to write and send prescriptions to a participating pharmacy through electronic means. The technology discourages the use of handwritten as well as faxing prescriptions. Although consensus on the impacts of e-prescribing, preliminary assessment of the city’s mental healthcare project issues an indication that it will be instrumental.

Demographic Information
Joshua tree is a city in California. The city has a population of 7,657 people. The city has a population density of 193 people per square mile. This is 25% less than the population density of California but it is 113% higher than the national average (World Population Review, 2020).

There are several challenges that have contributed to the seemingly low population in the city of Joshua Tree, California. One of the reasons is the relatively high rates of crime in the city. In this city, 1 in 59 people stands a chance of being involved in property crime. Besides, one in 232 people stand a chance of being involved in violent crime. To get a picture of the rates of crime in the city, evidence suggests that the city of Joshua Tree is only safer than 27% of the cities in the United States (Neighborhood Scout, 2020). The interpretation of this data is that the city of Joshua Tree among the cities in the United States with the worst levels of crime. The average age of the residents of the city of Joshua Tree is 41 years. This average age is 15% lower than that of California that is at 36 years. A significant majority of the population speaks English (87%) while 9% speak Spanish (World Population Review, 2020). On racial composition, 80% of the residents of the city of Joshua Tree are white while 1.92% is of Asian descent. African Americans from the minority group at 0.76%. There are more females than males in the city of Joshua Tree. For every 100 women, there are 92 men.
Types and locations of medical services
The city of Joshua Tree has a fairly advanced healthcare system. There are many primary healthcare organizations in the city of Joshua Tree. These healthcare organizations offer primary healthcare to the residents of the city. Primary healthcare providers are also known as primary care physicians. In the city of Joshua Tree, most of the primary care physicians offer outpatient and emergency services. One of the most unique features of the city of Joshua Tree's healthcare is that many of the healthcare professionals operate in freelance without being attached to any healthcare organization. Most of these professionals have small clinics where they offer healthcare services to their clients. Many families in this city identify with their family doctors they often call on them when they are in need of healthcare services. However, there are also many healthcare organizations that provide primary medical services to their clients. One of the most outstanding primary healthcare providers in the Hi-Desert Medical Center. This facility has some of the best medical services in the region. The Family Health Center is another significant medical organization in the region. This organization is equipped with some of the latest medical technologies and that provides it with the capability of addressing a wide range of medical services.
There are referral healthcare facilities in the city of Joshua Tree. These facilities largely handle medical issues that may not be addressed by primary healthcare providers. For example, when patients with adverse health complications are presented to primary healthcare providers but they realize that they are not capable of addressing such challenges. Some of the most prominent referral facilities in the city include Robert E Bush Naval Hospital and FCPP Multispecialty Clinic-Palm Springs. These facilities are equipped to provide effective care services to the clients.
Just like any other region, there are specific healthcare complications that characterize the population of the City of Joshua Tree. Most of the health complications that characterize the population include diabetes, blood pressure, and heart complications. Thus, any attempt to improve the quality of healthcare in the city must consider this uniqueness. One of the weak links in medical care in this region is the inadequacy of preventive interventions. In the recent past, most medical practices have been taking different approaches to healthcare and shifting focus on prevention. While there are many health challenges associated with the lifestyles, it is notable that there is little effort that is being made to improve on this health challenge. Lifestyle changes are likely to play an important role in making sure that the residents of the city of Joshua Tree enjoy good health.
Substance abuse is another healthcare challenge facing the residents of the city. Many of the young people residing in the city use drugs such as alcohol, marijuana, and cocaine. Overindulgence in some of these substances has contributed to the development of mental health challenges on the victims (Lajos & Zoltan, 2014). In response to the mental health challenges facing the residents of the city of Joshua Tree, many mental health facilities have been developed to handle the challenges. Some of them include Mental Health Systems Yucca Valley Center for Change, Mental Health Systems Inc. Indio Center for Change, Ranch Recovery Centers Inc. Hacienda Valdez and Desert Treatment Clinic among others.

An organizational chart is a visual presentation that indicates the reporting relationship and chain of command in an organization (Ahmady, Mehrpour, & Nikooravesh, 2016). The proposed mental healthcare organization in Joshua Tree City will be organized into the structure, as shown above. At the pinnacle, as usual, will be the city's Director of Health whose responsibilities include the overall management of healthcare within the city. The office of the director is supported by a local authority governing council charged with the responsibility of formulating health policies in the city. The council also coordinates with other health councils in the county or in the state to implement state or federal health outlines (Huguenin, Henry, & Ryan, 2018). Below the council is four-division broad departments that are responsible for various functions as outlined by the governing council. For instance, there is the Essential Records Division, Home Healthcare Division, Public Health and Medical Emergency Services.
The Mental Health Sub-Division is the proposed project which falls under Home Healthcare Division and is expected to be headed by a Practice Manager and organized into various units such as the one for nurses, medical assistants, care coordinators and front desk receptionist. As earlier observed the creation of this sub-department would not only look at the treatment of mental health complications in the city of Joshua Tree but also consider the most suitable mechanisms to address combat substance abuse rather than focusing only on treatments (Smith et al., 2014). The development of strategies to combat substance abuse is likely to be much more beneficial than the utilization of treatment approaches only.
